치과의사는 일상의 임상에서 예지성 높은 보철 수복치료를 하기위해 최선의 노력을 하고 있습니다. 그러나 실제 임상에서 하나의 치아에 문제가 발생된 경우, 이를 그 치아 하나만의 문제로 판단하고 간단히 보철치료를 한다면 치료결과가 오래 유지되기 어렵습니다.
생리적 기능과 비생리적 기능이 발현되는 악구강계 (치아, 치열, 악관절과 근신경계를 총칭) 전체를 진단하는 폭넓은 시야가 필요한 이유입니다.
Single crown치료의 결과가 오래 유지되기 위해 필요한 기본조건인 preparation 과 인상 및 바이트 채득, provisional restoration 제작의 중요성, 그리고 교합조정을 치료교합이라는 관점에서 이해해보고 싶습니다.
치료교합은 동적인 상태에서는 전치에 의한 구치이개를 통하여 과도한 측방압으로부터 구치의 위치와 형태를 보호하는 역할을 수행합니다.
구치의 위치와 형태가 유지되면 교두감합위에서 하악위를 안정시켜 다시 전치의 위치와 형태를 보호합니다.
우리는 이것을 상호보완교합(mutually protected occlusion)이라고 배웠습니다.
이러한 기본지식을 바탕으로 임상에서 실천한 증례를 통하여, 좁은 범위의 보철 수복치료 시 교합진단의 중요성을 이해하고 싶습니다.
The dentist is making every effort to provide a prominent prosthetic restorative treatment in daily clinical practice. when a problem occurs in one tooth, it is difficult to maintain the treatment result for a long time if it is judged that only one tooth problem and treated simply by prosthetic treatment. This is a reason we need a wide field of view to diagnose stomatognathic system (teeth, dentition, TM joints and neuromuscular system) in which physiological and non-physiological functions are expressed. I would like to understand the importance of tooth preparation, impression taking, bite registration, preparation of provisional restorations, and occlusal adjustment from the viewpoint of therapeutic occlusion. Therapeutic occlusion protects the position and shape of the molar from excessive lateral pressure through anterior guidance in dynamic condition. Once the position and shape of the molar is maintained, the position and shape of the anterior teeth is protected by stable ICP. We have learned that this is a mutually-protected occlusion. Based on this basic knowledge, I would like to understand the importance of occlusal diagnosis in a narrow range of prosthetic restorations through clinical cases.
