Congenital missing of permanent teeth affects some orthodontic patients. In treatment planning for orthodontic patient with congenital missing teeth, the orthodontist must decide whether 1) to keep the missing tooth space and replace it with prostheses or 2) close the space with orthodontic  treatment. In such a determination, the basic goals of orthodontic treatment, such as the facial balance and harmony, functional efficiency, stability of dentition, and healthy supporting tissue, should be considered. If the space will be left open for prosthetic treatment, the keys during orthodontic treatment are to create the correct amount of space and to leave the alveolar ridge in an ideal condition for a future restoration. If the space will be closed, the orthodontist must avoid any harmful changes to the occlusion and the facial esthetics. Therefore, the correct decision must be made at the appropriate time. In this presentation, I would like to present a several clinical options for treating orthodontic patients with congenitally missing teeth.
