진료를 하면서 계륵 같은 치아들을 참 많이 만난다. 뽑으려고 하니 조금 아깝기도 하고 환자도 거의 다 망가진 치아에 왜 그리도 집착을 하는지… 그렇다고 살리려고 하니 노력이 많이 필요할 것 같고 또 오래 쓸 수 있을지 잘 모르겠다.
치과의사 초년 시절 이런 치아들을 접하면서 많은 고민을 했다. 그럴 때마다 이리저리 물어도 보고 책도 찾아봤다. 뾰족한 답은 없지만 이리저리 근거들을 찾으며 나름 protocol을 잡아봤다.
이런저런 치료를 하면서 환자들이 치아에 대한 집착이 얼마나 큰지를 알았다.
환자에게 잠시나마 위로도 되고 구강도 다음진료를 하는데 좀 더 좋은 환경이 될 수 있는 중간단계의 처치도 환자에게 꽤 도움이 되겠다는 생각으로 시작했는데 결과가 꽤 좋다.
조금씩 주위에 소문이 나더니 급기야 발표를 할 수 있는 기회까지 생겼다.
이제는 진료를 할 날이 얼마 남지 않은 치과의사생활의 후반기를 살면서 후배들에게 뭔가 남기고 떠날 것이 있나 곰곰이 생각하며 이 주제의 강의를 준비했다. 모쪼록 후배분들께 조금이나마 참고가 되었으면 좋겠다.
In our daily clinical practice, we may encounter compromised teeth in many cases. Some are worth to save but some are not. Everyone here might have an experience about patients who are eager to maintain their teeth although tooth extraction is strongly recommended.
Early years as I faced compromised teeth, I was trying to find the critical evaluation factors and the best treatment to those teeth. I have searched lots of Books and scientific literature and discussed with other clinicians to build a predictable and reliable protocol to treat a compromised tooth based on scientific evidence.
Having given various treatment to the patients, I became to understand how big their expectations to keep their teeth. This treatment plan not only meets patients’ desires and satisfaction but also reduces potential risks associated with the tooth saving decision under unfavorable circumstance. Through this treatment the patients get better oral health and as a result it can lead to better outcome.
Thankfully, this idea gets a lot of support from other clinicians and finally I got an opportunity to present this.
Retention of compromised tooth, as apposed to tooth extraction, is one of the most difficult and multifactor-dependent decisions that dental professionals must make.
There are no absolutes or universal rules that can be applied to every case. However, I hope this lecture which is essence of my 30-year experience as a clinician, enlighten other clinicians in making the best decision to save or extract a tooth.
